Neat error coin here currently on eBay (buy-it-now for 150 EUR - seems excessive).
It seems the engraver probably cut the right star first (working left-to-right on die), then the central chi-rho, then lost track of what he was doing and started on another chi-rho rather than a star ! He seems to have realized his mistake half way though and given up trying to fix it.
